The Scotts Valley / San Lorenzo Valley Soccer Club would greatly appreciate receiving your feedback to assist us in evaluating the performance of our coaching staff for the Fall 2008 season. This information will be used to assist us in making coaching appointments for the Spring/Fall 2009 season. Your comments and evaluation will be held in the strictest confidence and will only be reviewed by the coaching coordinators and Club President for the Recreation Division, and the D3 Coordinator and Club President for the Competitive Division. Please submit your responses no later than November 15th, 2008.
