The applicable sections of the form need to be filled out completely.
All required signatures must be on the form in order for it to be processed.
The Club Registrar is to handle all player transfers / adds / or drops.
Signature Area
On the form, the team officials are the team coaches.
The designated "League" official for the Santa Cruz County Youth Soccer League
(for signature purposes) is the League Registrar.
